Informed Consent, Liability Waiver & Release Agreement

Effective Date: January 29, 2026

This Informed Consent, Liability Waiver, and Release Agreement ("Agreement") is entered into by and between Craft Peptides, LLC, a Texas Management Services Organization ("Craft Peptides," "we," "us," or "our"), its affiliated healthcare providers, and the undersigned patient ("Patient," "you," or "your"). By signing below, you acknowledge that you have read, understand, and voluntarily agree to all terms contained herein.

1. Nature of Services

Craft Peptides facilitates telemedicine consultations between patients and independently licensed healthcare providers. Our services include virtual medical consultations, the prescribing of peptide therapy protocols, and ongoing patient support. You understand and acknowledge that:

  1. (a) All medical services are provided by independently licensed physicians and physician assistants, not by Craft Peptides directly;
  2. (b) Telemedicine consultations occur remotely via electronic communications and do not involve in-person physical examination;
  3. (c) Peptide therapy involves the use of bioactive compounds that may produce various physiological effects; and
  4. (d) The decision to prescribe any treatment remains at the sole discretion of your healthcare provider.

2. Telemedicine Consent

You hereby consent to receive healthcare services via telemedicine. You understand that telemedicine involves the electronic transmission of personal medical information. You acknowledge the following limitations of telemedicine:

  1. (a) The inability to perform a hands-on physical examination;
  2. (b) Potential technology failures that may interrupt or prevent consultations;
  3. (c) Possible delays in medical evaluation and treatment due to technical difficulties;
  4. (d) Security protocols notwithstanding, electronic communications may be intercepted; and
  5. (e) Telemedicine is not appropriate for all medical conditions or emergencies.

3. Risks of Peptide Therapy

You understand that peptide therapy, like all medical treatments, carries inherent risks. While your healthcare provider will discuss specific risks related to your prescribed protocol, general risks may include but are not limited to:

  1. (a) Injection site reactions including redness, swelling, pain, or irritation;
  2. (b) Allergic reactions ranging from mild to severe;
  3. (c) Nausea, headache, dizziness, or fatigue;
  4. (d) Changes in blood sugar levels, water retention, or blood pressure;
  5. (e) Interactions with other medications or supplements;
  6. (f) Unknown long-term effects, as some peptides have limited long-term safety data; and
  7. (g) Other adverse effects not listed herein.

4. No Guarantee of Results

YOU EXPRESSLY ACKNOWLEDGE AND AGREE THAT CRAFT PEPTIDES AND ITS AFFILIATED HEALTHCARE PROVIDERS MAKE NO GUARANTEES, WARRANTIES, OR REPRESENTATIONS REGARDING THE OUTCOME OR EFFECTIVENESS OF ANY TREATMENT.

Individual results vary based on numerous factors including genetics, lifestyle, adherence to treatment protocols, and pre-existing health conditions. No specific outcomes have been promised to you, and you are not entering into this Agreement based on any such promises.

5. Patient Responsibilities

You agree to:

  1. (a) Provide accurate, complete, and truthful information regarding your medical history, current medications, supplements, and health conditions;
  2. (b) Follow all instructions provided by your healthcare provider regarding administration, dosage, and storage of prescribed treatments;
  3. (c) Immediately report any adverse reactions, side effects, or changes in your health status;
  4. (d) Attend all scheduled follow-up appointments and respond to provider communications;
  5. (e) Seek emergency medical care for any urgent or life-threatening symptoms; and
  6. (f) Not share, sell, or transfer any prescribed medications to others.

6. Assumption of Risk

You voluntarily assume all risks associated with the use of peptide therapy and telemedicine services. You acknowledge that you have been given the opportunity to ask questions about the proposed treatment, its alternatives, and its risks, and that your questions have been answered to your satisfaction. You understand that you may withdraw your consent and discontinue treatment at any time.

7. Release of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, you hereby release, waive, discharge, and covenant not to sue Craft Peptides, LLC, its officers, directors, managers, members, employees, agents, affiliates, affiliated healthcare providers, successors, and assigns (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by you, or to any property belonging to you, arising from or in connection with your use of our services, EXCEPT to the extent caused by the gross negligence or willful misconduct of the Released Parties.

This release includes, but is not limited to, claims arising from: (a) adverse reactions to prescribed treatments; (b) failure to achieve desired results; (c) technology failures or interruptions; (d) unauthorized access to electronic communications; and (e) any acts or omissions of third parties, including pharmacies and shipping carriers.

8. Indemnification

You agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising from: (a) your breach of this Agreement; (b) your misuse of prescribed treatments; (c) your provision of false or misleading information; or (d) your violation of any applicable law or regulation.

9. HIPAA Acknowledgment

You acknowledge that you have received, reviewed, and understand our Notice of Privacy Practices, which describes how your protected health information may be used and disclosed. You consent to the use and disclosure of your health information as described therein and as permitted or required by law.

10. Limitation of Liability

IN NO EVENT SHALL THE RELEASED PARTIES BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, REGARDLESS OF THE CAUSE OF ACTION OR WHETHER SUCH DAMAGES WERE FORESEEABLE.

To the extent any liability is found, the total cumulative liability of the Released Parties shall not exceed the total fees paid by you to Craft Peptides during the twelve (12) months preceding the claim.

11. Governing Law & Dispute Resolution

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law principles. Any dispute arising out of or relating to this Agreement shall be resolved through binding arbitration administered by the American Arbitration Association in Austin, Texas, in accordance with its Commercial Arbitration Rules. The arbitrator's decision shall be final and binding, and judgment may be entered in any court of competent jurisdiction.

12. Severability

If any provision of this Agreement is held to be invalid, illegal, or unenforceable, such provision shall be modified to the minimum extent necessary to make it valid and enforceable, or if modification is not possible, shall be severed from this Agreement. The validity and enforceability of the remaining provisions shall not be affected.

13. Entire Agreement

This Agreement, together with our Terms of Service and Privacy Policy, constitutes the entire agreement between you and Craft Peptides with respect to the subject matter hereof and supersedes all prior or contemporaneous understandings, agreements, or representations.
