Telemedicine Services

Telemedicine Services Agreement

This Telemedicine Services Agreement ("Agreement") governs your use of telemedicine services provided through Craft Peptides. By using our telemedicine platform, you acknowledge that you have read, understood, and agree to be bound by the terms of this Agreement.

1. Definition of Telemedicine Services

Telemedicine involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. Services may include:

  • • Synchronous (real-time) video consultations
  • • Asynchronous (store-and-forward) communications
  • • Secure messaging through our platform
  • • Telephone consultations when clinically appropriate

2. Scope of Services

Craft Peptides provides telemedicine consultations for peptide therapy and related wellness services. Our licensed healthcare providers will:

  • • Review your medical history and health questionnaire
  • • Conduct virtual consultations to assess your health needs
  • • Develop personalized treatment plans when clinically appropriate
  • • Prescribe medications, including peptides, when medically indicated
  • • Provide follow-up care and ongoing monitoring

3. Benefits of Telemedicine

Telemedicine offers several potential benefits, including:

  • • Convenient access to healthcare from your location
  • • Reduced travel time and associated costs
  • • Flexible scheduling options
  • • Timely access to specialist consultations

4. Limitations and Risks of Telemedicine

Technical Limitations:

  • • Technology failures may interrupt or prevent services
  • • Poor image or audio quality may affect the consultation
  • • Security protocols may fail despite best efforts

Clinical Limitations:

  • • Physical examination is limited or not possible
  • • Diagnostic testing cannot be performed remotely
  • • Some conditions may not be appropriate for telemedicine evaluation

5. Patient Responsibilities

As a patient using telemedicine services, you agree to:

  1. (a) Provide accurate, complete, and current health information
  2. (b) Be located within a state where our providers are licensed at the time of consultation
  3. (c) Ensure you have adequate technology and internet connection for video consultations
  4. (d) Be present in a private, quiet location during consultations
  5. (e) Actively participate in your care and follow treatment recommendations
  6. (f) Contact emergency services (911) for any medical emergencies

6. Emergency Situations

Telemedicine services are NOT intended for emergency situations.

If you experience a medical emergency, including but not limited to chest pain, difficulty breathing, severe allergic reaction, loss of consciousness, or any life-threatening condition, you should:

  • • Call 911 immediately
  • • Go to your nearest emergency room
  • • Contact your local emergency services

Do NOT rely on telemedicine services for emergency medical care.

7. Privacy and Confidentiality

We are committed to protecting your health information in accordance with HIPAA and applicable state privacy laws. Our telemedicine platform uses encryption and security measures to protect your information.

8. Prescriptions

If your healthcare provider determines that a prescription is appropriate, prescriptions will be sent electronically to your designated pharmacy or compounding pharmacy. You understand that:

  • • Not all medications can be prescribed via telemedicine
  • • Controlled substances may have additional restrictions
  • • You are responsible for using medications as directed
  • • Prescription decisions are at the sole discretion of your healthcare provider

9. Right to Refuse or Withdraw

You have the right to refuse telemedicine services at any time without affecting your right to future care. You may also request to stop a telemedicine consultation at any time.

10. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions.
